Question Number 1:
Under the Freedom of Information Act 2000, I would like to request information regarding clinical documentation practices, specifically in relation to the capture of information during interventional procedures, such as Central Venous Access Device (CVAD) insertions.

Clinical Documentation

• What methods are currently used by clinicians to document key information during invasive procedures, such as CVAD insertions (e.g., paper forms, digital forms, voice dictation, other)?

o Is any form of structured digital data entry used at the point of care for these procedures?

• Are standardised templates or proformas used Trust-wide for documenting invasive procedures, such as Local Safety Standards for Invasive Procedures (LocSSIPs)?

o Are any of these LocSSIPs or other forms digitised?
 
Answer To Question 1:
Response from Radiology:
-paper documents scanned onto Radiology Information System and ‘scan for safety’
-Radiology Information System
-Both NatSSIPs and LocSSIPs
-No
Response from Anaesthetics/ICU & Theatres:
We currently use paper forms to document our invasive procedures, including an Audit form.
Is any form of structured digital data entry used at the point of care for these procedures - not at present, awaiting role out of EPR.
There are LoCSSIPS (under ICU) on ELOAS to be used across the trust for
CVAD insertion
NG insertion
Bronchoscopy
Tracheostomy
Chest Drain
Intubation
Are any of these LocSSIPs or other forms digitised - not currently digitised, however they will be on EPR rollout.

Response from Cardiology :
A. Cardiology procedures are documented in medical notes if an in patient or a letter to GP is generated. Documentation also transcribed to CVIS.
B. Yes CVIS
C. Cannot comment trust wide but yes for Cardiology.
D. On CVIS
 
Question Number 2:
Voice Technology

• Does the Trust currently use any voice-to-text or voice recognition technology for clinical documentation (e.g., Dragon Medical One, Nuance, Voice-Care, etc.)?

• If yes, which departments or specialties currently use it, and for which types of documentation?
 
Answer To Question 2:
VR is only used within Radiology currently to report on imaging

Response from Radiology: Yes Dragon in Radiology

Response from Cardiology - N?A to cardiology per se but not in our department.
 
Question Number 3:
Administrative Efficiency

• Has the Trust undertaken any assessments of the administrative time burden associated with completing procedural documentation (manual or digital)?

o If so, is any information available on time spent per procedure or efforts to streamline the process?

 
Answer To Question 3:
Response from Anaesthetics/ICU and Theatres:
We have not looked at administrative time, however when using the LoCSSIP all procedural information is completed on the reverse of the form. So that there is no need to duplicate information.

Response from Cardiology - yes and yes.
 
Question Number 4:
Future Plans

• Is the Trust currently planning any initiatives or procurements to digitise documentation processes for interventional procedures?

o If yes, please outline the scope and timelines if available.
 
Answer To Question 4:
Considering opportunities for Trustwide voice recognition and ambient AI. timescales and scope currently to be confirmed.
